DHI hair transplant
Dhi (handle) hair transplantation was developed by Yang Choi in the early 1990s and is a widely used method of graft implantation, especially in Far East Asia. There are many names for this method, such as Choi implant pen, hair implant pen, feather hair transplant, no touch hair transplant (hair transplant without touching the hair follicles).
The pen method has become widespread in our country in recent years and with it the wrong name is the DHI hair transplant method. In fact, DHI is the initials of the company Direct Hair Implant, which is engaged in hair transplantation in Greece. The company performs hair transplantation with the Choi pen technique. Actually there is no method called DHI. However, we also use the term in connection with its current perception and the idea behind it.

Stages of hair transplantation using the Dhi method (with a pen)
The implant pen we use in every hair transplant is a small mechanical device that works with a spring system similar to a ballpoint pen. Its outer layer is made of Teflon and can withstand approximately 130-140 degrees of sterilization. At the end is a needle into which grafts are placed. The needle is very similar to a syringe, but the needle has slots vertically. They serve to attach hair follicles to them. For fast and synchronous operation, on average, 4-6 handles and implants are required.
a) Placement of the hair roots in the handle. Hair follicles distributed in Petri dishes are placed one at a time at the end of the needle by the physician's assistants. During the installation process, the root part of the graft is never touched, the hair is held by the upper part with forceps and inserted into the needle by sliding. The follicles should not remain in the pen for long, otherwise the roots may dry out. If necessary, assistants moisten them with isotonic solution.
b) Exchange of empty and finished pens. During the exchange of empty and full pens, coordination between the doctor and his assistants must be perfectly established. Accidents that can lead to needle stick injury should be kept to a minimum. How to achieve this? The doctor places an empty pen on the table with the tip of the needle towards him. The assistant returns the filled pen to the table parallel to the table and the doctor. Between full and empty handles, the distance is at least 30-40 cm. Thus, DHI hair transplantation is fast and the staff works in sync with the doctor. There is another method of transplantation, where only the doctor's wrist is involved. The doctor's hand is fixed from the elbow. One of the assistants is in front of the doctor, he replaces empty pens with full ones, putting them directly into the doctor's hand. The doctor at one time holds a full pen between thumb and forefinger. This allows DHI hair transplantation to be performed much faster. In addition, the doctor gets tired less, as hand movements are minimized.
c) Hair implantation by a doctor. The doctor brings a needle with a hair inside at an appropriate angle to the area of baldness, the tip of the needle pierces the skin, the doctor presses the clamping part at the top of the handle. A thin wire extended from the tip of the needle is pressed against the top of the hair follicle, the hair is pushed inward. Thus, the graft is placed in the area of baldness. The tip of the needle is removed from the skin with a spring system immediately, completing the implantation of the graft. During transplantation, the patient's neck should not be strained. Otherwise, bleeding may increase due to pressure in the vessels.
The doctor should comfortably hold the handle. The 3rd, 4th and 5th fingers squeeze and support it from below and behind. The thumb wraps around the front of the handle. Thus, the filled pen is immersed in the skin. As the pen penetrates the skin, the graft should be planted synchronously. To do this, the doctor presses the index finger on top of the handle, leaving the follicle in the skin. Once it is implanted into the skin, the handle is quickly removed. When the doctor performing the procedure fully adapts to this rhythm, Dhi hair transplantation is quick and effective.
