    What is hair transplant?

    Hair transplantation is a surgical practice that consists in the transfer of one's own hair with its root, unharmed and with its annexes , from an area of ​​the head, defined as the donor area, to another bald or thinning area, which we define the recipient area . The hair transplanted to their new location will behave as they would have done in their original site , giving rise to normal growth.


    This now codified method was described in 1939 by a Japanese doctor Dr. Okuda and perfected in all its details by Dr. Orentreinch in 1959. Over the years, numerous scholars have further perfected this technique in order to improve the aesthetic result and make it as natural as possible.


    The hair of the donor area is taken from the occipital and temporal regions (posterior and lateral) of the patient's head; in these areas , in fact, even in very extensive baldness, we will all be able to notice how hair does not fall out due to the diversity of hormonal receptors that characterize it. In practice, even people with very extensive and inveterate baldness retain a strip of hair that goes from the upper edge of one ear to the other and which will be useful for carrying out the surgery.


    It is therefore clear that hair transplantation redistributes existing follicles and that it cannot create new ones (hence the use of the term "autotransplant"). Therefore the number of hair that each of us has available is fixed.


    Why is a hair transplant performed?

    Hair transplantation turns out to be the only lasting and effective solution to androgenetic alopecia .


    How is the autograft performed?

    Modern hair transplantation techniques allow you to regain hair in bald or thinned areas that grow regularly and do not require any other immediate and time-consuming treatment, other than ordinary washing and periodic shortening.


    Currently the most used techniques to perform the autotransplant are two: the FUE and the FUT .


    FUE is a technique that involves hair transplantation through direct removal of the follicular units (“tear” technique) with consequent small scars. With this technique, the various grafts are removed from the posterior and lateral regions of the head (donor area) by means of a hollow cylindrical scalpel (punch), which can frequently lead to damage to the follicles . The removal of the hair, however, is carried out "blindly", in the sense that the surgeon cannot see the real orientation of the hair inside the scalp, which could cause damage to the bulbs, in addition to the fact that the extraction is carried out "by pulling ”Being able to damage and break the hair roots.


    The follicular units, more or less traumatized, are then inserted directly into the affected part to be thickened through small incisions.


    FUT, considered the gold standard of surgical techniques, consists instead in the removal by the surgeon of a strip of scalp (STRIP) from the posterior or lateral regions of the head. The strip is then placed under special microscopes and divided by highly specialized personnel (doctors, biologists, etc.) into follicular units under the direct visual control of the surgeon, with practically no possibility of damage. The surgeon will then make micro-incisions in the bald or thickening areas, in which he will insert the hair to be transplanted.

    There is an advanced method applied to FUT , called CFU .

    How does the intervention take place?

     Hair transplant surgery with the FUT technique is not painful and is performed under local anesthesia associated with mild sedation in the presence of an anesthetist.

    An intervention with the FUE technique can take place over several days with sessions of up to 7-8 hours, while an intervention with the FUT technique takes place in a single day and lasts between 4.5 and 5.5 hours.


    In any case, it is necessary to carry out a series of blood tests and a control electrocardiogram before the surgery in order to verify the good state of health of the candidate for surgery, so that the doctor can evaluate if there are any contraindications.


    The tests to be performed are therefore typically: ECG, blood count, PT PTT, azotemia, creatininemia, electrolytes (Na, K, Cl), fibrinogen, glycemia, transaminases, Ab. HCV, HbsAg, HIV, blood group.



    What happens after the operation?

    Most patients may experience a “tight” sensation in the donor area (behind the neck) for 1-2 days. The numbness that can sometimes occur in the donor or recipient area usually disappears within a few weeks or months of surgery.

    After the intervention on each "tagliettino" a small crust will form. Once healing has taken place, the crust will come off leaving a pink mark that will tend to lighten in a short time.

    With the crust, even the hair embedded in it may fall out, but you don't have to worry: from the receiving area, after about three months, the new transplanted hairs will begin to appear, which will grow with the rhythm and methods similar to those of their home origin.



    How often can hair transplant be done?

    Hair transplantation can also be repeated more than once. This is in relation to the size of the bald area or to the eventual evolution of baldness in the untreated areas over the years.


    Results and expectations

    It should be noted that the surgeon does not create hair, he redistributes it, so the results will depend on the width of the bald area and the goodness of the donor area . What makes the difference is the ability of the experienced surgeon to have high engraftment rates , which means that it is not important how many hairs are transplanted but that all the hair that is transplanted takes root.


    In some cases, results are obtained that can be spectacular, but there are no standard interventions, there are patients, each with their own expectations and possibilities for correction. This is why it is very important not to identify with the results that we can see on other patients both in a positive and in a negative sense. Each patient is a story in itself , because, if it is true that the transplanted hair remains for life, it is equally true that the loss of residual hair in the untreated areas can be chased for many years or for a lifetime .

    Furthermore, it is necessary to take into account the donor area, which can be more or less thick, more or less extended and more or less elastic, in addition to the quality of the hair which, according to its type, with the same result, can give different results. A fine hair yields less than a thick hair, a thick hair yields less than a curly hair. Another variable is given by the chromatic tone of the hair which less contrasts with the underlying light skin, the more it gives a good effect of thickness of the transplant.


    It is therefore necessary to plan the intervention well and verify if the expectations of the candidate are realistic in relation to the age, the donor area available and the bald area to be treated.



    How long do the results of a hair transplant last?

    As previously mentioned, the transplanted hair is not destined to fall out , since those coming from the donor area are genetically different and we could define them as "permanent". This hair will never die, so the hair transplant result will last forever in the treated area. This does not mean that the progression of baldness of residual hair could continue over the years, attacking only the non-transplanted hair thus modifying the result obtained, which can be improved with a possible new transplant.


    Who can not have a hair transplant?

    Hair transplant is not recommended:



    In patients with unrealistic expectations in relation to their degree of baldness, it is often significant.

    In those patients who have serious health problems, which do not allow to carry out any surgery in full safety conditions. The latter includes patients suffering from serious pathologies or autoimmune diseases . For this reason, the patient will be asked to carry out a series of blood tests and a control electrocardiogram before surgery.
